The Wellness Home Behavioral Health organization provides outpatient consumer mental health services for adolescents and adults from age 12 and up who are affected by mental illness. Wellness Home Behavioral Health Services are person-centered, family-centered, trauma-infromed, recovery-oriented, services provided through a whole person approach (mind, body, and soul). Collaboratively, Wellness Home Behavioral Health Services and the Wellness Homes initiative offers advanced integration of behavioral health with preventative primary care. The Wellness Home model utilizes evidence-based practices that are team-oriented to improve access to high quality wellness and health services. The model employs care coordination to organize activites and share information among all relevent participants.
